Vaccinations Required for Lake Local Seniors before School Starts in Fall of 2017
Immunization requirements for school attendance in Ohio have changed, requiring that all Lake seniors receive a meningitis vaccine booster before school starts Thursday, August 17, 2017.
For convenience of parents and incoming seniors, a vaccine clinic will be offered to Lake High School students Wednesday, March 15, 2017 from 10 a.m. - noon in the high school clinic; completed screening information and insurance forms must be returned to the Lake High School Clinic by March 10, 2017.
Payment will be based on the student’s insurance coverage. Rite Aid will verify your insurance coverage and contact you with the price you will need to pay for the vaccine prior to the date of the clinic.
A letter to incoming seniors and required screening/insurance forms are now available both online and in the Lake High School Clinic.
A second immunization clinic will be offered Thursday, June 1, 2017 from 10 a.m. – 2 p.m. in the Lake High School main gym. Completed screening information and insurance forms must be returned to the Lake High School Clinic by Tuesday, May 30, 2017.
All incoming seniors are now required to receive the Meningococcal (A, C, Y, W-135) vaccine (to protect against Meningitis) before the 2017-2018 school year begins. If your student received a Meningitis (A, C, Y, W-135) vaccine before the age of 16, a second dose is required for school attendance in twelfth grade. If your student will be receiving the first dose of Meningitis (A, C, Y, W-135) vaccine and is now 16 years of age or older, only one dose is required for school attendance during twelfth grade.
Any twelfth grade student who has not had the required Meningitis (A, C, Y, W-135) vaccination will not be permitted to attend school with the following exception:
A pupil who presents a written statement of the pupil's parent or guardian in which the parent or guardian declines to have the pupil immunized for reasons of conscience, including religious convictions, is not required to be immunized. Ohio Revised Code section 3313.67.
The Meningitis immunization can be obtained at any time from now until the first day of twelfth grade. We also encourage you to contact your Primary Care physician, pharmacist, or the Stark County Health Department for an appointment.
Don’t wait! Clinics tend to become very busy toward the end of summer, causing a delay in immunization which could result in exclusion from school.
As soon as these required immunizations are obtained, please provide a copy of your child’s shot record, including the Meningitis vaccination dates, to Lake High School so your student’s record can be updated.
