Immunizations Required for Incoming Seventh Graders, Seniors before Sept, 4, 2018
All incoming Lake seniors and seventh graders are required by Ohio school law to show proof of receiving immunizations BEFORE the first day of school next fall, which will be Sept. 4, 2018 for grades 1-12.
The immunizations are:

(Seventh grade)
* TDaP vaccine (to protect against Tetanus, Diphtheria and Pertussis)
* Meningococcal (A, C, Y, W-135) vaccine (to protect against Meningitis)

* Meningococcal (A, C, Y, W-135) vaccine (to protect against Meningitis)

Any student who has not had the required vaccinations will not be permitted to attend school until proof of these immunizations or alternative requirements according to Ohio Revised Code section 3313.67 have been received by the school.  

Both the required TDaP and Meningitis (A, C, Y, W-135) shots can be obtained at any time from now until the first day of school.
As a convenience to Lake parents, the meningitis vaccine  will be offered to students ages 16 and older through two clinics to be held at Lake High School (LHS), provided by Rite Aid Pharmacy.  The two dates and times of the clinics are:
Thursday, May 3, 2018 10:30 a.m. – 11:30 a.m. in LHS Clinic  
(Please return required, completed screening questionnaire and insurance information forms to the LHS Clinic by Monday, April 30, 2018.)     
Tuesday, June 5, 2018 9 a.m. – 12 noon in LHS Main Gym     
(Please return required, completed screening questionnaire and insurance information forms to the LHS Clinic by Friday, June 1, 2018.)

Payment for LHS Clinic Immunizations: 
• Payment will be based on insurance coverage. The price of the vaccine with no insurance coverage is $163.99. Rite Aid will verify your insurance coverage and contact you with the price you will need to pay for the vaccine.

• Payment will be due at the clinic (cash or check made payable to Rite Aid.) 

If you have questions, please contact Lauren, the pharmacist at Rite Aid Pharmacy in Canal Fulton at 330-854-6618, or Therese Gilbert, Lake Local School Nurse, at 330-877-4282.
We also encourage you to contact your Primary Care Physician, local pharmacy, urgent care center, or the Stark County Health Department for an appointment if your student is younger than 16 years of age, or these two clinic dates are not convenient.

When required immunizations are obtained, please provide a copy of your child's shot record, with the vaccination dates, to the Lake Middle School or LHS main office after August 20, 2018 so your student's record can be updated.

Note exception: A pupil who presents a written statement of the pupil's parent or guardian in which the parent or guardian declines to have the pupil immunized for reasons of conscience, including religious convictions, is not required to be immunized.  Ohio Revised Code section 3313.67.
