Lake High School Immunization Clinic is Set; Return of Student Medications Required
The next opportunity for incoming fall 2017 LHS seniors to get a required meningitis vaccination will be Thursday, June 1, 2017, from 10 a.m. - 2 p.m. in the Lake High School Main Gym. State law now requires that this booster dose of the meningitis vaccine be completed in order for students aged 16 and older to attend school.
All students entering twelfth grade in the fall of 2017 will be REQUIRED to show proof of receiving a Meningococcal (A,C,Y,W-135) vaccine (to protect from Meningitis) by the start of the 2017-18 school year.

Lake students wishing to be immunized June 1, 2017 should return necessary completed forms to the LHS Clinic by Tuesday, May 30, 2017.  The required screening questionnaire and insurance information forms are downloadable from the district website, or hard copies can be picked up in the high school clinic.

Payment will be based on insurance coverage. The price of the vaccine with no insurance coverage is $153.99. Rite Aid will verify your insurance coverage and contact you with the price you will need to pay for the vaccine. Payment will be due at the clinic (cash or check made payable to Rite Aid).

A pupil who presents a written statement of the pupil's parent or guardian in which the parent/guardian declines to have the pupil immunized for reasons of conscience, including religious convictions, is not required to be immunized. Ohio Revised Code section 3313.67.

End of Year Medication Pick Up
In addition, the district also requires that parents make arrangements to pick up medications used by their children during the school day before the current school year ends. Parents are asked to go to their children’s respective school clinic(s) on or before the last day of school (Wednesday, May 31, 2017) and pick up any remaining medications.

All medications not picked up by 3 p.m. the last day of school will be disposed of. No further phone calls will be made.
